外眼及角膜重點備忘筆記

1. Bowman's layer不是basement membrane，不屬Type 4 collagen

2. central cloudy dystrophy of Francois對視力不影響，病患VA佳

3. Tetracycline不會有aplastic anemia的副作用（chloramphenico, CAI會有aplastic anemia的副作用）

4. 有AMBD及cataract者，可用RGP來over-refract以得到visual potential評估目的。

5. alergic conjunctivitis通常會有papillary conjunctivitis而非follicular conjunctivitis

6. 外眼用fluorescein無毒、可溶於水，可偵測微細的表皮不規則病徵。但沒有antiviral特性（又不是卡巴斯基，當然不能防毒囉！）。

7. corneal stroma的RI=1.376，而不是1.336（房水的RI）。
=> with-the-rule astigmatism(WTR)會呈現長軸在vertical的ellpse

8. conjunctival goblet cell產出的mucin鋪蓋在cornea上，對其上的aqueous為hydrophilic，對其下的corneal epithelium為hydrophobic
=> MGD, rosacea會造成decrease TBUT (<10秒異常）

9. 用哪種手術「移除」pterygium不重要，更重要的是「減少recur的機會」
=> 可採用下列方法：free conjunctival graft, mitomycin-c, beta irradiation，皆有效果。

10. 當tearfilm osmolarity減少時，代表aqueous tear的量增加，相對稀釋了solute的濃度。
=> lysosome, lactoferrin隨著tear分泌，濃度減少時則代表aqueous tear deficiency。

11. sarcoidosis最重要的corneal finding是calcific band keratopathy

12. keratoconjunctivitis sicca在下午、晚上會比早上更嚴重（愈晚愈乾愈糟糕）
=> ABMD, Fuchs endothelial dystrophy, floppy lid syndrome在一早起床時症狀最明顯。

13. chronic alcoholism, cystic fibrosis, bowel resection皆可造成xerophthalmia(Vit. A deficiency)
=> glumerulonephritis不會造成xerophthalmia

14. Pseudomonas aeruginosa不會穿透intact cornea
(N. meningitidis C. diphtheriae, Shigella可穿透inact cornea)

15. adenovirus不能用alcohol殺死（要用dilute bleach for 10mins以上）

16. Acanthamoeba的infectious form只有trophozoite。而dormant cyst也可在人類身上發現。
=> Acanthamoeba可在buffered charcoal-yeast agar上生長
=> 可被calcoflour white, acridine orage, Giemsa stain染色
=> 在早期可呈現pseudodendrite的樣子

17. ocular adenoviral infection「不會」造成enlarged corneal nerves（Acanthamoeba， MEN2b, leprosy … 才會有）

18. 根據HEDS,對於已使用topical corticosteroids及trifluridine的HSV stromal keratitis患者，再使用oral acyclovir therapy沒有額外的助益
=> 短期內，oral acyclovir無法避免HSV stromal keratitis/iritis的發作
=> 長期而言，oral acyclovir可以避免HSV stromal keratitis的recurrence並保存視力
=> topical corticosteroids與prophylactic antivial一起治療HSV stromal keratitis，不但可減少發炎，也可縮短病程。
（口訣：一種藥加一種藥可能有效。兩種藥再加一種藥，藥太多就好像沒什麼幫助了。）

19.HZO的治療，使用systemic antiviral therapy (topical無效）
=> oral famciclovir 500mg TID
=> valacyclovir 1000mg TID
=> acyclovir 800mg PID(五次） for 7~10 days
★ 對於disseminated HZO危險群或immunosuppression者可用IV form acyclovir

20. Vernal keratoconjunctivitis不會有symblepharon

21. GVHD的主要表現之一是keratitis sicca, 也可能有cicatrical lagophthalmos.
=> GVHD常發生在BMT之後
=> active nonocular GVHD與ocular surface diseae有關

22. interstitial keratitis(IK)雖然多由congenital syphilis造成，但HZV,HSV, C. trachomatis也是原因之一。

23. limbal cell dysfunction與aniridia, contact lens wea, atopic keratoconjunctivitis等原因有關。

24. Lattice, Granular, Avelino, Reis-Bucklers corneal dystrophies與BIG3 gene有關，但Macular dystrophy屬閒雜人等，當然無關。

25. squamous cell carcinoma的excision要在tumor margin 4mm範圍以外的conjunctiva下手。

26. oculodermal melanocytosis有5%是雙側，造成uveal melanoma的機率是1/400，比正常人的1/13000高出不少。

27. PAM出現malignant transformation的徵兆： nodularity, enlargement, increased vascularity.（口訣：惡性「大結血」 …會不會是良性「大長今」的妹妹？）

28. topical藥物沒有teratogenic effect。maternal diabetes有teratogenic effect。

29. nanophtalmos的眼球小而美（functional），若要放IOL，因為眼軸短，所以要選度數「特高」的IOL。

30.CHED1發病時間在1-2歲左右，所以不像CHED2有corneal clouding at birth（CHED2是congenital glaucoma的D/D，而CHED1不是）

31. lattice dystrophy在PK之後recurrence機會很高

32. PPMD的corneal endothelium會像endothlium，有mirovilli,多層的endothelium，其間以intracellular desmosome相連（不是hemidesmosome)。=> PPMD與PAS造成angle-closure有關。PK後容易recurrence。

33. Schnyder dystrophy只有50%可看到central subepithelial crystal。週邊會有arcus lipoides，另有midperipheral corneal opacification，corneal sensation也會降(Dr. Schnyder一直強調這一點！）。

34. "elevated" srum phosphorus with normal serum calcium是造成band keratopathy的原因之一。

35. Gaucher disease「不會」造成cornea verticillata
=> chlorpromazine可造成cornea verticillata(不是chloramphenicol）
=> chloroquine可造成cornea verticillata
=> Fabry disease可造成cornea verticillata

36. 藥物造成cornea verticillata，觀察即可，不需停藥。

37. herpetic disease, retinoblasoma, juvenile xanthogranuloma皆可造成spontaneous hyphema

39. limbal stem cell deficiency是慢性且漸進的疾病，通常不會自己好。（aniridia, chemical burn, severe inflammation, chronic contact lens wear都會引起。）

40. conjunctival flap不是治療ocular perforation的好方法，仍然應該要seal起來再說。HSV stromal keratitis可以用conjunctival flap來治療。

41. post. choroidal melanoma可以是cornea donor

42. megalocornea在corneal topography顯示normal curvature
=> dry eye 會有irregular astigmatism，在topo下可表現出來。

43. small wound leak而無AC shallow => 3天內不要resuture

44. radial perineuritis不是GVHD的signs

45. atopy是造成bilateral herpes simplex最常見的原因。
